Thank you for considering St. Jerome Early Childhood Center, a NAEYC accredited program, for your child! Our application/re-enrollment process for all students is outlined below for the 2018-19 school year. (If you are in need of application materials for the remainder of this current school year, please call the ECC office, 727-596-9491.)
The following materials are required to complete the re-enrollment/registration process: Application/Re-enrollment Form (for each child with both parent (or legal guardian) signatures. Original Birth Certificate (for new students. A copy will be made in the office) $175.00 Application/Supply/Student Insurance Fee (Non-refundable) OR for 4 year-olds – An application fee of $100.00 and a VPK Program: Certificate of Eligibility and the St. Jerome ECC VPK Parent Agreement
Please click the required forms below, fill in the appropriate information, print, and submit the items to the ECC Office. Unfortunately, due to licensing and other constraints, we do not store or accept data electronically at this time. You may have the option to save a copy or save as you work if your software version allows. This will save it to your personal computer (not the school computer) for your records. Please do not actually submit and email the forms! We need the printed copy, with appropriate signatures in pen, delivered to the school office. Thank you.
Student forms to completed by July 1st will be added here by May 18 for all who have submitted and application form/fee. Additional forms & items will be distributed during Family Orientation Night in August. Each form is necessary for various reasons, licensing, student file, teacher file, class setup, etc. We appreciate your efforts in completing all necessary paperwork in a timely manner!
