Please review the two crucial dates of July 1 and August 1 for records and forms below.  Also, please review the additional Back to School documents that are not time sensitive.

Note: We are excited to offer this information on-line.  Please click the required forms, fill in the appropriate information and print them out to turn in to the school.  Unfortunately due to licensing and other constraints, we do not store data electronically at this time.  You may have the option to save a copy or save as you work if your software version allows.  Pleas note, this will save it to your personal computer (not the school computer) for your records.  The forms must be printed and delivered to the school office for your child's file. OR, hand write the information on your printed copy or obtain paper copies at the school office.

q Physical and Immunization forms obtained from your child's doctor

q Medical Support Plan (if applicable) Needed for all children who require medication to be administered at school or to have emergency medications on hand at school. No medication may be kept at school without these forms, which include the doctor’s written medication plan.

q IEP for any students will special needs.

q LAST CHANCE – CHILD MAY NOT ATTEND SCHOOL WITHOUT
